there should already be a method called 'Request a Quote'.
Click on that to open it, then in the box at the top click the 'Enabled' checkbox.
Scroll further down and select the countries that you want it to apply to.
I've got something in the back of my mind telling me that you must have at least 1 shipping method configured so buyers can use instant checkout... but I can't find anything so I could be wrong on that.
