Submitting feedback for order is not working

I'm not able to submit feedback for an order. Tried it both from the order page and from the orders list page.

The submit feedback page loads correctly, but upon submission it just reloads looking the same, and checking back on the the order I can see that feedback wasn't submitted.

I've inspected the network traffic with the browser developer tools and I can see the POST request on that page returns a successful 200.
Also, no errors in the console.

Had this happen before. Turned out the user in question was no longer registered, but it didn't give any indication that that was the problem.

We did have an issue after updating the form system, so that may have been the cause at that time. If you try again, that should now work.
